MRI Safety Screening and Liability Disclaimer

(California Compliance Notice)

Patient Safety Notice

Magnetic Resonance Imaging (MRI) uses powerful magnetic fields and radiofrequency energy. Certain implanted medical devices, metallic objects, or medical conditions may create serious safety risks during an MRI examination, including device malfunction, movement of metallic objects, severe injury, or death.

In accordance with recognized MRI safety standards and California medical safety practices, all patients must complete a safety screening prior to undergoing an MRI examination.

Contraindicated Devices and Conditions

For patient safety, this MRI facility does not perform MRI examinations on patients with the following implanted devices unless the device has been confirmed in writing as MRI-safe or MRI-conditional by the device manufacturer and verified by a physician:

• Cardiac Pacemakers
• Implantable Cardioverter Defibrillators (ICD)
• Neurostimulators or implanted stimulators
• Brain aneurysm clips
• Artificial heart valves (unless documented MRI-safe)

If a patient has any of the above devices and appropriate documentation confirming MRI compatibility cannot be provided, the MRI examination will not be performed.

Patient Disclosure Responsibility

Patients are required to fully disclose all medical implants, devices, surgeries, injuries involving metal, or any other medical condition that may affect MRI safety.

Failure to disclose complete and accurate information may pose serious medical risks. This MRI facility relies on the information provided by the patient, referring physician, and available medical documentation when determining eligibility for an MRI examination.

Right to Refuse or Postpone Examination

This MRI imaging center reserves the right to refuse, delay, or cancel an MRI examination if:

  • Patient safety cannot be confirmed
  • Implant compatibility cannot be verified
  • Required medical documentation is not provided
  • MRI safety screening is incomplete

These actions are taken solely to protect patient safety and comply with established medical safety standards.

Limitation of Liability

This facility performs MRI examinations in accordance with standard MRI safety practices and applicable California medical regulations. Determination of MRI eligibility is based on information provided by the patient and referring medical provider.

The facility shall not be responsible for complications, injuries, or adverse outcomes resulting from incomplete, inaccurate, or withheld medical information provided by the patient or referring provider.

Medical Oversight

All MRI procedures are performed under the supervision of qualified medical personnel and in accordance with established MRI safety protocols recognized by the medical imaging community.
